Completed McGowan Allied Specialty Insurance Application.
Completed ACORD forms.
Supplemental Apps (New business App, Bowling Center App. Roller Rink app. and other supplements as applicable).
Currently valued Hard Copy Loss Runs for past 5 years. Provide details of any large losses ($25,000 or higher) and advise what the prospect has done to prevent future similar losses.
A copy of the waiver used (if applicable).
Within the email submission, provide a summary of the operations, losses, target and prior pricing, and other key points from your field underwriting review.
Pictures are always helpful (include diagram of facility).
